Position: Practice Medical Director
Lead the strategic transformation toward value-based care models as our Practice Medical Director. Drive innovative initiatives within accountable care organizations (ACOs) and bundled payment programs, guiding our team toward improved patient outcomes and cost-effective healthcare delivery. Oversee interdisciplinary collaboration, empowering advanced practice providers, case manager nurses, and care manager social workers to excel in delivering high-quality, patient-centered care. - Provide comprehensive clinical care for patients with multiple comorbidities including CKD primarily via telehealth with occasional in-home visits.

Roles & Responsibilities
- Oversee population health management initiatives, including risk stratification, care coordination, and patient engagement strategies to improve health outcomes and reduce healthcare disparities.
- Utilize data analytics and performance metrics to track and monitor key quality and cost indicators, identify areas for improvement, and drive continuous quality improvement.
- Treat patients as the direct provider in collaboration with APPs and interdisciplinary team, providing evidence-based care including complex care through pharmacological and non-pharmacological interventions.
- Lead the development and implementation of clinical guidelines, protocols, and best practices to standardize care delivery and promote evidence-based medicine.
- Collaborate with executive leadership, clinical teams, and external stakeholders to develop and implement value-based care strategies that improve patient outcomes, reduce costs, and enhance the overall quality of care.
Position Requirements
- Medical degree from an accredited medical school.
- Board certification in Internal medicine, geriatrics, or family medicine.
- Active medical license and DEA registration in at least one state and willing to get licensed in multiple states in the region.
- Minimum of 3 years of clinical practice experience, with demonstrated leadership and management experience in healthcare delivery or administration.
- Excellent analytical skills, with the ability to interpret data, analyze trends, and make data-driven decisions.
- Exceptional communication and interpersonal skills, with the ability to effectively engage and influence stakeholders at all levels of the organization.
- Demonstrated ability to lead change, drive innovation, and implement complex healthcare initiatives in a dynamic and fast-paced environment.
